To enroll in the Medicinal Cannabis Program, patients must have a qualifying medical condition and be under the care of a participating health care provider. If you and your health care provider think you will benefit from adding cannabis to your course of treatment, they must provide you with an Authorizing Healthcare Practitioner Statement. The statement will include a reference ID number and a registry ID number, which you will need to establish an account in the patient portal and complete a registration process.
Your patient account will allow you to track your application status, verify your allotment and authorization period, request ID cards, and send messages to Patient Services.
To set up your account on the patient portal and to complete registration, you will need the following:
Medicinal cannabis patients visiting from outside of New Jersey who wish to register for a temporary card will only need a reference ID number and registry ID number from a New Jersey authorized health care practitioner and a current copy of their registration in their home state's program to set up a patient account.
